Patient Selection Criteria for TheraPoint's Ambulatory Infusion Centers...
• Patient’s therapy must be appropriate to the outpatient setting and must be ordered by an appropriately licensed and credentialed physician.
• Patient MUST be medically stable with the risk of significant changes in condition to be considered low.
• Patient’s therapy must be able to be delivered during regular hours of operation. (Monday through Friday, 8:30 a.m. to 4:30 p.m., closed on holidays). If a patient is unable to keep an appointment during those hours TheraPoint staff will not be available to administer therapy after hours and it will be the patient’s responsibility to make other arrangements.
• Patient must be able to SAFELY enter, leave and move about the AIC (Ambulatory Infusion Center) on their own or with the assistance of a family member or caregiver. If assistance from a family member or caregiver is needed then a family member/caregiver must accompany the patient with EVERY visit to the AIC.
• Patient must be independent with personal care needs and use of the restroom or be able to do so safely with the assistance of a family member/caregiver.
• If patient requires the use of assistive devices such as canes, walkers, wheelchairs, etc. the patient will be responsible to provide their own devices.
• Patient is responsible for providing their own, safe transportation to and from the AIC, keeping in mind that their therapy may make it inappropriate for them to drive themselves home afterwards.
• Because the safety of our patients is our highest priority, TheraPoint reserves the right to discontinue services to any patient if the clinical staff deems that changes in patient condition, or any other factors noted, render the patient inappropriate for treatment in an outpatient AIC environment.
